Please read and fill-out this form to register your child for POWER CAMP 2019.
POWER CAMP is a Christian Day Camp with a sports emphasis at Tuscarora Inn and Conference Center in Mount Bethel, Pa. The Camp is for children who have just completed 1st grade thru 6th grade. Each day, campers will be dropped-off at the Auditorium between 8:30-9:00am and picked-up at 3:30pm. The day will consist of open recreation, large-group sessions and games, a focused sports clinic, lunch, small-group time, team competitions and guest speakers.
The clinics offered will be basketball, soccer, flag football and an arts and crafts option. Please designate below which clinic your child would like to participate in for the week. All clinics require appropriate attire, including footwear. NOTE: Soccer clinic will require shin guards and a soccer ball (cleats optional)
Lunch will be bag-lunch. In addition to food and drinks for lunch, please plan on sending a refillable water bottle. NOTE: Please do not pack peanut-butter or foods containing nuts.
The registration cost for POWER CAMP is $125. POWER CAMP is a non-profit fundraiser. The proceeds of POWER CAMP go towards Mt. Bethel Youth Ministries.
POWER CAMP offers sports clinics as part of the daily schedule. Please choose the clinic in which they plan to participate for the week.
An on-site nurse will be available all week to administer first-aid. In the event of an emergency, please tell us who you would like us to call.
Each day, your child will need to be signed-in at 9 am and signed-out at 3:30, by an adult. Please list any names and numbers of approved adults, other than the Parent/Guardians listed above, with whom your child may be released:
My son/daughter is a voluntary participant in POWER CAMP 2019 and is in good health. I recognize the possibility of a physical injury associated with a sports camp and in consideration for Mount Bethel Church allowing my son/daughter to participate in this camp, I release and hold harmless Mount Bethel Church, including affiliated individuals and organizations, Tuscarora Inn and Conference Center, POWER CAMP organizers, sponsors, and all camp volunteers and staff from any and all claims that may arise from participation in this event including all personal injuries. Additionally, I grant POWER CAMP volunteers to act as my surrogate for my child in the area of obtaining medical treatment by a medical doctor, dentist, nurse, or athletic trainer. I also assume full financial responsibility for any medical or dental treatment for my child. I understand that I and my emergency contacts will be contacted immediately by the POWER CAMP staff through the telephone numbers that I have provided in case of any medical or dental problems with my child and assure that I will be available immediately to make medical decisions for my child. Additionally, I grant permission for photographs of my child to be taken during this camp and used only without his/her name in POWER CAMP and Tuscarora publications.

POWER CAMP is run by Mount Bethel Church and much of the staff is volunteer. Please help us by encouraging others to serve as Team Leaders and staff. Please feel free to share this material with any who might be interested. We need volunteers to serve as team leaders, coaches, administrators and much more!
On the evening of Thursday, June 20th, POWER CAMP will be hosting a family pot-luck at Mount Bethel Church from 6:00-8:00pm. Friends, family and siblings are invited to attend this after-dinner program where we will hear from the campers about their week at POWER CAMP.
The cost for POWER CAMP is $125.00 per child, after May 31st.

Scholarships may be available, if needed.
