This consent form is for Jr. High students to be able to be transported from the Middle School to Mount Bethel Church Youth Group functions, including, but not limited to: Youth Group at Mount Bethel Church, Tuscarora, Dollar General, Kelly's Ice Cream and Cici's Pizza.

By checking the box below, I recognize this form as my digital signature and thereby give consent for my child to drive with an approved bus, van, or personal vehicle deemed appropriate by Mount Bethel Church for transporting my child.
